  • Submit a COI with the following info listed:

    Astoria, NY 11103

    Requirements:
    - $1,000,000 General Liability
    - Minimum of $50,000 Inland Marine coverage for Misc. Rented Equipment (varies per project)

  • Submit tax exemption form ST-121 if available. (Optional)
